1. Choose which type of CSA course you would like to attend from the following:

-Full day individual bookings £350/delegate [maxium of 3 delegates/group]

-Full day one to one teaching with Dr Raj Thakkar - £895

-Full day group booking [maximum 3 delegates] - with group discount - £950

2. Email your requests to Raj Thakkar at gpst3courses@gmail.com

3. Send a cheque made payable to Dr R Thakkar to:

You will receive a confirmatory email upon receipt of your cheque. You may cancel with a full refund up to 10 days prior to the course date.
